Emergency Information Sheet

One page that helps a neighbour, relative or responder act quickly: who to call, which hospital is preferred, and a few things worth knowing. Fill in only what is useful.

Nothing is saved or sent. What you type stays in this browser tab and is gone when you close it. Print the sheet, or leave it blank and write on it by hand.

Keep copies private. A printed sheet contains personal details. Keep it where trusted people can find it, such as a folder near the phone, rather than on display to visitors. In India, 112 is the national emergency number.

You can print it blank and fill it in by hand.

About the person

What they like to be called.

Helps an ambulance or neighbour find the home.

Who to call

Health contacts

The provider's name and helpline are usually enough. Keep policy documents separately.

Key care information

Only what the person and family are comfortable writing down.

Point to the list rather than copying it here, so there is only one version to update.

For example: wears hearing aids, uses a walker, needs glasses to read.
